After watching several Boys and Girls matches over the last few weeks, what makes a ref decide between a direct or indirect kick?

That's the problem, little if any consistency between refs. One never knows what the call will be, due to the way the refs see the game, and interpret the rules.
I'm afraid that's the nature of the beast. There are a lot of subjective judgement calls in soccer. Notice the phrase "...in a manner considered by the referee to be careless, reckless or using excessive force." For example, charging in itself is not a foul. However, it is a foul to charge in a manner that is careless, reckless, or using excessive force. And, of course, that is all dependent upon the "opinion of the referee".
